Moving to Malta: residence, healthcare and living costs

Keep Malta’s residence permission and special tax status separate while building a suitable housing, healthcare and spending plan.

Malta: international living and wealth planning

Residence permission and tax status

Start by establishing the right to live in Malta. Work-based permissions, family routes and the Malta Permanent Residence Programme serve different circumstances. Do not confuse the MPRP immigration route with the MRP retirement tax program; their similar initials can hide different requirements.

Then establish residence, domicile and any special tax status. Confirm which family members are covered, what absences are permitted and which approvals must be renewed. Review the 2027 special-tax framework separately from the conditions of an immigration permit.

Country facts to keep in view

Residence routes
Work / permanent residence programsThe MPRP grants permanent residence to qualifying third-country applicants. A tax scheme and immigration permission are separate.
Citizenship
5-year residence route; discretionaryTypically 12 months immediately before applying plus four years in the preceding six, with additional conditions. Residence does not guarantee citizenship.
Healthcare
Public entitlement or private insuranceEmployment, eligible S1 and reciprocal routes can provide cover. Programme applications may require private insurance.
Property ownership
Purchase permit may be requiredNationality, residence and the property's location affect AIP permit requirements.

Programme commitments and daily spending

A Malta budget should distinguish ordinary living costs from commitments attached to a chosen program. Rent or purchase conditions, insurance, application charges and minimum tax can make a particular route more expensive than its headline rate suggests.

Compare locations and housing suitable for the household, allowing for transport, summer cooling and trips abroad. Confirm public healthcare entitlement, any eligible S1 route and any separate insurance required by the program. The lifestyle allowance should not be mistaken for the minimum cost of maintaining special tax status.

Adjust the lifestyle budget

Confirm healthcare access before relying on it

Employment, eligible S1 and reciprocal routes can provide cover. Programme applications may require private insurance. Public provision, insurance and out-of-pocket charges need separate checks. Obtain a quote for your age, medical history and required cover; the budget tool is an allowance rather than an insurance quotation.

Residence and citizenship are separate decisions

Typically 12 months immediately before applying plus four years in the preceding six, with additional conditions. Residence does not guarantee citizenship. Check how your chosen permit counts towards any later application and what absences are allowed. Buying a property or paying an application fee is not an assurance that citizenship will follow.

Before leaving or returning to the UK

Identify the particular UK pension and proposed receipt under the UK–Malta convention, including remittance-related conditions and government-service provisions. Separately review the UK departure and the Maltese classification of ISA or brokerage income. A remittance from a UK account is not automatically capital simply because the account predates the move.

US connections continue to matter

The US–Malta treaty’s pension definition does not cover every Maltese personal arrangement. Read the competent-authority clarification before funding a new structure. Keep US retirement accounts separate and check PFIC treatment before buying non-US investments; Maltese program status does not settle US obligations.

BRING THESE QUESTIONS

Make the conversation useful.

  • Which immigration permission and tax status would I hold, and what must I maintain?
  • What is the full annual commitment of the Malta program alongside my lifestyle budget?
  • Which commitments should I delay until permission and healthcare are confirmed?
